[tei-council] Unfinished <quotation> element in example?

I've fixed the <quotation> problem, but monogr.xml still has 
non-deterministic content model issues. I tried one fix on the first 
bit, but rolled it back when it didn't solve the problem. I'll come back 
to it tomorrow.
