[tei-council] notes in web guidelines

I have updated the pages at http://www.tei-c.org/release/doc/tei-p5-doc/en/html/ and points south so that
they include headers.

I propose to let sleeping dogs lie now as regards the bug Stuart found, and let the other copies
of the HTML files in eg Debian remain uncorrected.
