Do we have the final proposal on what pointers should look like? is 
SOW-09 it? as I understand it,
<xref> and <xptr> vanish, and the "target" attribute of <ref> and <ptr> 
points to a URL. Correct?

And I assume this is part and parcel of the change to xml:id, or do we 
do them separately?
