[tei-council] New Prefix Definition Proposal (was "Private URI Schemes")

Thanks Kevin. I have a draft of a Guidelines section already done, and a 
couple more bits of Glines changes to write and add to the proposal 
before I ask for the final go-ahead. Should be done by next week.
