[tei-council] travel reimbursement form

You can find slightly different versions of the travel reimbursement
form at

The former one has slightly larger type, and is therefore a bit
easier to use, but the latter one is a bit easier to get. The only
real difference I noticed was the address to which to send the form,
which is in Norway, and therefore both are now wrong. I believe the
correct address would be
