I've added the Proust example from Elena, and made a bit more revision 
of the section on "Aspects of layout" aiming to make it a bit less 
ignorant of the rest of the chapter. In the process I moved <gb> to the 
core, along with <pb> and <cb>.

I think that really is it for this chapter, unless anyone identifies 
anything really urgent in the next day or so.
