[tei-council] update web guidelines

I regenerated P5 1.9.1 with revised XSL, and have got back the missing notes, live at eg http://www.tei-c.org/release/doc/tei-p5-doc/en/html/HD.html

Unfortunately, the banner is lost for reasons I do not understand so I will have to do it all again.

I have also updated the epub version, as I found a curious error which mean that all lower-level section heads were duplicated.
