I'm afraid this draft is a shocking mess, considering the time it took me
to get it out to you. I apologize. In the future I'll restrain myself from
volunteering so glibly.

Please send corrections to me. One specific request: DoD (or someone anyone
else who got it right), could you please supply the missing information
currently marked by <!-- SOMETHING -->?

After everyone has had a chance to chime in I'll send to David, who will
put the file in its final resting place.
