[tei-council] printed guidelines (again)

my final attempt before Christmas.  I have hopefully dealt
with the problems reported (and, maybe, introduced new ones).

I am still concerned about margins if its printed on US lettersize
paper. Until I am sure those are right, I cannot look at overruns
into the margin.

Chris R reports printing successfully, but it needs definite
decisions about printing (if any).
