I agree that some sort of warning is needed on this point, but it should 
not take the form of saying that the content of the <fw> dont form part 
of the surrounding div, since they clearly do, in some sense! They are 
contained by that div, just as the <fw> itself is, and just as *all* the 
children of (say) a <choice> element within that <div> would be, or a 
<desc> child of some element that used to have a @desc attribute. You 
might even want to process them with relation to the div, for example in 
the case of a running head which changes to reflect the content of the 
page.  But I agree that a warning about the need for careful processing 
is in order, particularly in the context of the comparison with <pb/>
