[tei-council] formatting decisions implemented after consulting oracle

- the "bookmark link" is now at the end of the section heading, as a pilcrow
- empty elements are marked with a trailing /
- the class appendices are presented with AtoZ lists like elements
- AtoZ lists have headings for each section
