Here's my solution:
<!-- is there a more elegant solution for the <note>-
element? -->
<!-- <bibl xml:id="PH-BIBL-1">
<title level="a">The Manere of Good Lyuynge</
title>, fol 126v of Bodleian MS Laud Misc.
517, plate 8(ii) in <title>English Cursive Book
Hands 1250– 1500</title> by M. B. Parkes
(Clarendon Press: Oxford, 1969)</bibl> -->
<biblStruct xml:id="PH-BIBL-1">
<analytic>
<title level="a">The Manere of Good Lyuynge</title>
</analytic>
<monogr>
<author>
<forename>M.</forename>
<forename>B.</forename>
<surname>Parkes</surname>
</author>
<title level="m">English Cursive Book Hands 1250– 1500</title>
<imprint>
<pubPlace>Oxford</pubPlace>
<publisher>Clarendon Press</publisher>
<date>1969</date>
</imprint>
</monogr>
<biblScope type="plate">8(ii)</biblScope>
<note>Facsimile of fol. 126v of Bodleian MS Laud Misc.
517</note>
</biblStruct>
The changes are: Parkes is the monographic author, not the analytic; the
plate number goes in biblScope, and the fact that the image is a
facsimile of f. 126v of the MS is treated as a bibliographic note to the
whole item.
